From 17 February 2020, your medical record will be able to be viewed by other health and social care professionals directly involved in your care via a ShareforYou record.

Please visit www.pennineshareforyou.nhs.uk/ for more information. If you would prefer that we didn’t share your record then you can opt out at any time.
